Kindergarten Registration Information

BURRILLVILLE KINDERGARTEN REGISTRATlON for Burrillville children entering Kindergarten in September 2010

WHEN: Tuesday, February 23, 2010 from 4:00pm and 8:00pm AND Thursday, February 25, 2010 from 4:00pm and 8:00pm

WHERE: Austin T. Levy School, 135 Main Street

Child must turn 5 by September 1st, 2010

You must have the following to register:

• Birth Certificate
• Immunization Record and Physical
• Proof of Residency is required as follows:
  
  • Two (2) current utility bills dated within 1 month of registration
  • Current tax bill -OR- Mortgage statement
  • Notarized affidavit affirming residency (form will be provided at registration or downloaded below)

All requested information must be provided before the student starts school.

Children may be screened at Levy on registration day or by calling Child Outreach at 401-647-7560 to make an appointment for one of the following dates: February 2^nd, February 23^rd, March 12^th or April 13^th.
