Kindergarten Registration Information

BURRILLVILLE KINDERGARTEN REGISTRATlON for Burrillville children entering Kindergarten in September 2011

WHEN: Tuesday, April 10, 2012 from 4pm to 8pm AND Thursday, April 12, 2011 from 4pm to 8pm

WHERE: Austin T. Levy School, 135 Main Street

Child must turn 5 by September 1st, 2012

Parents or Guardians who reside in Burrillville and wish to register students in the public schools of Burrillville MUST complete the Student Registration form, which can be downloaded below.

You MUST have the following to register:

  • Birth Certificate
  • Immunization Record and Physical
  • PROOF OF RESIDENCY is required as follows:
    • Affidavit affirming residence; notarized statement of residency (form to be provided at registration OR can be downloaded below)
      • IF you rent and landlord is unable to sign the affidavit, a copy of the lease or rental agreement WITH the landlord's signature may be provided
    • Two (2) current utility bills (electric, cab;e, gas, telephone, etc.) dated within 1 month of registration
    • Current Burrillvile tax bill -OR- Mortgage statement

Please note:

  • A driver's license IS NOT an acceptable proof of residency
  • A post office box IS NOT an acceptable address
  • If the registrant is staying in a residence rented or owned by someone else, then the registrant must ask the renter/owner to sign a notarized letter, including evidence of ownership/lease agreement, and naming the adult and the student(s) staying there. The letter must be brought to registration with other proof of residency.

All requested information MUST be provided BEFORE the student starts school.

Parent Notification of Kindergarten
Health Requirements

Dear Parent/ Guardian,

State law requires that any child entering kindergarten must have documented evidence of the following:

1. Immunizations:

  • DTP (Diphtheria, Tetanus, Pertussis) 5 doses
  • IPV/ OPV (polio) 4 doses
  • Hepatitis B 3 doses
  • MMR (measles, mumps rubella) 2 doses
  • Varicella 2 doses or proof of chicken pox disease.

2. Physical examination:

  • must be dated within 1 year of the start of school.
  • Proof of vision screening by a licensed provider.
  • Proof that lead screening was done.

Please note that any information missing from your child’s health record will result in registration delay for your child. Your child will not be allowed to enter school until all necessary information is received.

If you have an appointment scheduled to meet compliance with the above requirements, notify me of the physician name and scheduled date so that I may contact them for verification. Please contact me for any questions, concerns, or difficulties you may be have obtaining the required information prior to the end of school in June. All information submitted over the summer will be reviewed prior to the start of school.
